The undersigned , in making application to the Metrolina Native American Association for membership, swear (or affirm), that I, the applicant named herein, am of American Indian descent or the spouse of a American Indian eligible for membership in the Association and that all information provided in connection with the understanding with this application is true to the best of my knowledge and belief, with the understanding that any omissions, inaccuracies or failure to make full disclosure may be considered sufficient reason to deny membership or to suspend membership after membership established. The undersigned applicant further authorizes Metrolina Native America Association to verify all information listen on this.
